AHDB: What is your general approach to treatment? Dr Allen: If my patient with rosacea has more bumps than erythema, I prescribe metronidazole gel, and if the patient has more erythema than bumps, I prescribe the 15% azeleic acid gel, because it is not as effective for bumps. Studies have shown that the 15% azeleic acid gel is more effective than the 20% azeleic acid cream, because it penetrates more deeply into the skin, and I have observed this in clinical practice. Some patients will need both metronidazole and azeleic acid, and others with more severe papules and pustules will require a systemic antibiotic. For these patients I prescribe doxycycline or minocycline.

AHDB: Are there new or emerging treatment options for rosacea?Dr Allen: For patients who have some seborrheic dermatitis associated with their rosacea (which is fairly common), I am starting to use hyaluronate acid sodium salt 0.2% (Bionect) cream, which attracts and binds water and plays a role in inflammation and tissue repair, as well as Promiseb cream, an antiseborrheic cream that helps to repair the skin barrier. These are alternatives to topical steroids, which can be good for rosacea flares but should not be used for more than a few weeks.

Note: Fraudulent online pharmacies may attempt to sell an illegal generic version of Azelex. These medications may be counterfeit and potentially unsafe. If you purchase medications online, be sure you are buying from a reputable and valid online pharmacy. Ask your health care provider for advice if you are unsure about the online purchase of any medication.

Skinoren cream is usually applied to the affected area twice a day, morning and evening after the area has been washed and patted dry. For people with sensitive skin, it can be used just in the evening to start with, for a week, before starting to use it twice a day. The cream comes in a tube and is applied to the skin with the fingertips. It should not be used by children under the age of 12.

Skinoren cream can take around four weeks before you will see it working; and if necessary, it can be used for up to a year continuously. It should be used regularly for the best results. For acne that does not respond after a month of treatment, or rosacea that does not respond after two months of treatment, talk to your doctor as Skinoren may not be the best option for you.

There are some situations where Skinoren cream should be used with caution or would not be recommended, so complete the medical questionnaire carefully. It is not usually used at the same time as any other treatments for acne. If it doesn't work for you, as above, talk to your doctor as a different treatment might be better.

Azelaic acid gel and foam is used to clear the bumps, lesions, and swelling caused by rosacea (a skin disease that causes redness, flushing, and pimples on the face). Azelaic acid cream is used to treat the pimples and swelling caused by acne. Azelaic acid is in a class of medications called dicarboxylic acids. It works to treat rosacea by decreasing the swelling and redness of the skin. It works to treat acne by killing the bacteria that infect pores and by decreasing production of keratin, a natural substance that can lead to the development of acne.

This medication comes in the form of a topical cream that is usually applied to the affected area twice daily or as directed by your doctor. The affected area should be cleaned with mild soap and dried with a towel before this medication is applied. Avoid covering the affected area after you have applied Azelex. Doing this may cause too much of the medicine to be absorbed by your body, which can be harmful.

Common side effects that may occur while using Azelex cream include burning, stinging, itching, or tingling of the skin where the medication was applied. You may also experience dryness, peeling, irritation, or redness of the skin. These effects may occur at the beginning of treatment and usually go away as your body adjusts to the medication. You should contact your doctor immediately if these effects persist or worsen.

Avoid contact in or near the eyes or inside the nose and mouth. If this medication gets into any of these areas, wash right away with plenty of water and call your doctor if irritation lasts. Do not place a bandage or cover over the cream unless your doctor tells you to do so.

Do not flush medications down the toilet or pour them into a drain unless instructed to do so. Properly discard this product when it is expired or no longer needed. Consult your pharmacist or local waste disposal company. Images Azelex 20 % topical cream

Azelaic acid is a saturated dicarboxylic acid found naturally in wheat, rye, and barley. It is also produced by Malassezia furfur, also known as Pityrosporum ovale, which is a species of fungus that is normally found on human skin. Azelaic acid is effective against a number of skin conditions, such as mild to moderate acne, when applied topically in a cream formulation of 20%. It works in part by stopping the growth of skin bacteria that cause acne, and by keeping skin pores clear. Azelaic acid's antimicrobial action may be attributable to inhibition of microbial cellular protein synthesis.

After the skin is thoroughly washed and patted dry, a thin film of AZELEX cream should be gently but thoroughly massaged into the affected areas twice daily, in the morning and evening. The hands should be washed following application. The duration of use of AZELEX cream can vary from person to person and depends on the severity of the acne. Improvement of the condition occurs in the majority of patients with inflammatory lesions within four weeks.

During U.S. clinical trials with AZELEX cream, adverse reactions were generally mild and transient in nature. The most common adverse reactions occurring in approximately 1-5% of patients were pruritus, burning, stinging and tingling. Other adverse reactions such as erythema, dryness, rash, peeling, irritation, dermatitis, and contact dermatitis were reported in less than 1% of subjects. There is the potential for experiencing allergic reactions with use of AZELEX cream.

Azelaic acid is a human dietary component of a simple molecular structure that does not suggest carcinogenic potential, and it does not belong to a class of drugs for which there is a concern about carcinogenicity. Therefore, animal studies to evaluate carcinogenic potential with AZELEX cream were not deemed necessary. In a battery of tests (Ames assay, HGPRT test in Chinese hamster ovary cells, human lymphocyte test, dominant lethal assay in mice), azelaic acid was found to be nonmutagenic. Animal studies have shown no adverse effects on fertility.

Equilibrium dialysis was used to assess human milk partitioning in vitro. At an azelaic acid concentration of 25 mcg/mL, the milk/plasma distribution coefficient was 0.7 and the milk/buffer distribution was 1.0, indicating that passage of drug into maternal milk may occur. Since less than 4% of a topically applied dose is systemically absorbed, the uptake of azelaic acid into maternal milk is not expected to cause a significant change from baseline azelaic acid levels in the milk. However, caution should be exercised when AZELEX cream is administered to a nursing mother.

A normalization of keratinization leading to an anticomedonal effect of azelaic acid may also contribute to its clinical activity. Electron microscopic and immunohistochemical evaluation of skin biopsies from human subjects treated with AZELEX cream demonstrated a reduction in the thickness of the stratum corneum, a reduction in number and size of keratohyalin granules, and a reduction in the amount and distribution of filaggrin (a protein component of keratohyalin) in epidermal layers. This is suggestive of the ability to decrease microcomedo formation. 041b061a72
